Visit by Age: Usual Procedures
- 10 days: Newborn metabolic blood screening test
- 1 month: May receive Hepatitis B
- 2 months: Hepatitis B, Pentacel (DTaP, IPV, HIB), Prevnar 13 (Pneumococcal), Rotavirus
- 4 months: Pentacel, Prevnar 13, Rotavirus and Hepatitis B if not received at birth
- 6 months: Pentacel, Prevnar 13, Rotavirus and Hepatitis B
- 9 months: Anemia and Lead Screens
- 12 months: Varicella (Chicken Pox), MMR (or MMRV combo), Prevnar 13, Hepatitis A, and Tuberculosis screening
- 15 months: Pentacel (DTaP, IPV, HIB)
- 18 months: Hepatitis A, DTaP (if not previously received), and developmental screening
- 2 years: Hepatitis A (if two doses not completed), lead screening, developmental screening
- 2 1/2 years
- 3 years
- 4-5 years: DTaP, IPV, MMR, Varicella (Combinations MMRV and DTaP-IPV available)
- 6-8 years: Well visits are scheduled yearly.
- 9-10 years: Well visits are scheduled yearly. Cholesterol and anemia screening
- 11-13 years: Well visits are scheduled yearly. Tdap, Meningococcal vaccine, HPV for both boys and girls, cholesterol and anemia screening
- 14-15 years: Well visits are scheduled yearly. HPV (if not previously given)
- 16-18 years: Well visits are scheduled yearly. Meningococcal booster, HPV (if not previously given), Meningococcal B, cholesterol and anemia screening
- 19-23 years: Well visits are scheduled yearly. Meningococcal booster, HPV (if not previously given), Tdap booster, cholesterol and anemia screening
