Visit by Age: Usual Procedures
- 10 days: Newborn metabolic blood screening test
- 1 month: Hepatitis B
- 2 months: Hepatitis B(if not given at birth), Pentacel (DTaP, IPV, HIB), Prevnar 13 (Pneumococcal), Rotavirus
- 4 months: Pentacel, Prevnar 13, Rotavirus
- 6 months: Pentacel, Prevnar 13, Rotavirus (Flu and COVID vaccines will be offered as well)
- 9 months: Hepatitis B, Anemia and Lead Screens
- 12 months: MMRV (Measles, Mumps, Rubella, Varicella) , Prevnar 13, Hepatitis A
- 15 months: Pentacel (DTaP, IPV, HIB)
- 18 months: Hepatitis A and developmental screening
- 2 years: Hepatitis A (if two doses not completed), anemia and lead screening, developmental screening
- 2 1/2 years: Developmental screening
- 3 years
- 4-5 years: MMRV and DTaP-IPV (Measles, Mumps, Rubella, Varicella and Diphtheria, Tetanus, Pertussis, Polio)
- 6-8 years: Well visits are scheduled yearly.
- 9-10 years: Well visits are scheduled yearly. Cholesterol and anemia screening
- 11-13 years: Well visits are scheduled yearly. Tdap, Meningococcal ACYW vaccine, HPV for both boys and girls, cholesterol and anemia screening
- 14-15 years: Well visits are scheduled yearly. HPV (if not previously given)
- 16-18 years: Well visits are scheduled yearly. Meningococcal ACYW booster, HPV (if not previously given), Meningococcal B, cholesterol and anemia screening
- 19-23 years: Well visits are scheduled yearly. Meningococcal B, HPV (if not previously given), Tdap booster, cholesterol and anemia screening
