Immunizations and Health Screening

Visit by Age: Usual Procedures

10 days: Newborn metabolic blood screening test

1 month: May receive Hepatitis B

2 months: Hepatitis B, Pentacel (DTaP, IPV, HIB), Prevnar 13 (Pneumococcal), Rotavirus

4 months: Pentacel, Prevnar 13, Rotavirus and Hepatitis B if not received at birth

6 months: Pentacel, Prevnar 13, Rotavirus and Hepatitis B

9 months: Anemia and Lead Screens

12 months: Varicella (Chicken Pox), MMR (or MMRV combo), Prevnar 13, Hepatitis A, and Tuberculosis screening

15 months: DTaP, HIB

18 months: Hepatitis A, DTaP (if not previously received), and developmental screening

2 years: Hepatitis A (if two doses not completed), lead screening, developmental screening

2 1/2 years

3 years

4-5 years: DTaP, IPV, MMR, Varicella (Combinations MMRV and DTaP-IPV available)

6-8 years: Well visits are scheduled yearly.

9-10 years: Well visits are scheduled yearly. Cholesterol and anemia screening

11-13 years: Well visits are scheduled yearly. Tdap, Meningococcal vaccine, HPV for both boys and girls, cholesterol and anemia screening

14-15 years: Well visits are scheduled yearly. HPV (if not previously given)

16-21 years: Well visits are scheduled yearly. Meningococcal booster, HPV (if not previously given), cholesterol and anemia screening
